What are subperiosteal dental implants?
Subperiosteal implants, also known as juxtaosseous implants, are an advanced solution for patients with severe bone atrophy who cannot receive conventional dental implants.
Unlike traditional implants, which are inserted into the bone, these are placed on top of the maxillary or mandibular bone, beneath the gum tissue, using a customised titanium structure.
Thanks to 3D planning technologies, intraoral scanners and CAD-CAM systems, the implant is custom-designed to fit each patient’s bone anatomy. This allows for precise, minimally invasive and highly predictable placement.
Who are candidates for subperiosteal implants?
This type of implant is particularly suitable for patients who have:
- Advanced bone loss due to resorption, ageing, or periodontal disease.
- Failure or inability to receive a traditional bone graft.
- Cases where aggressive surgery such as zygomatic implants is to be avoided.
- Patients who have been wearing removable dentures for years and are seeking a permanent fixed solution.

Do you suffer from bone atrophy and have you been told that you cannot have implants?
Bone atrophy is no longer an obstacle. Subperiosteal implants are an advanced, customised solution that does not require grafts, ideal for patients with significant bone loss.
Thanks to 3D planning and CAD-CAM technology, they allow a fixed prosthesis to be fitted from day one, even in the most complex cases.

Steps in Periostic Implant Surgery
-
We begin with a 3D CT scan, intraoral scan and comprehensive medical examination to assess bone quality and design the best personalised plan.
-
Based on the study, we create a biocompatible titanium structure adapted to the maxilla or mandible using high-precision CAD-CAM technology.
-
The implant is placed in the clinic on an outpatient basis under local anaesthetic or conscious sedation, without the need for hospitalisation.
-
Whenever possible, we place a fixed temporary dental prosthesis during the same procedure, restoring aesthetics and function immediately.
-
We establish a schedule of clinical and radiographic check-ups to ensure the treatment progresses correctly and to resolve any issues that may arise.
Care after the procedure
Soft, healing diet
During the first few days after the subperiosteal implant is placed, it is recommended to eat soft, cold, non-irritating foods to promote soft tissue healing and avoid trauma to the surgical area. Avoid hot, hard or acidic foods.
Post-surgical oral hygiene
Maintain strict hygiene by using an ultra-soft surgical toothbrush and antiseptic mouthwash with chlorhexidine (0.12% or 0.2%) as directed by your dentist. This helps prevent infections, control bacterial plaque and maintain perimplant gingival health.
Physical rest and inflammation control
Avoid physical exercise, heavy lifting or high-impact activities for the first 5 to 7 days. Rest promotes proper vascularisation of the implanted area and reduces the risk of bruising, inflammation or post-operative bleeding.
Beneficios de los Implantes Periósticos
-
They avoid complex bone regeneration techniques such as block grafting or sinus lifting, reducing surgical times and risks.
-
Procedure performed in the clinic, under local anaesthesia or conscious sedation, using a minimally invasive surgical approach.
-
Most patients resume their normal lives within 2-3 weeks, with minimal swelling and simple post-operative care.
-
It allows for immediate oral rehabilitation, with fixed teeth placed on the same day, without the need for temporary removable prostheses.
-
Thanks to CAD-CAM technology and 3D scanners, the implant adapts to the residual bone anatomy of the maxilla or mandible, improving precision and fit.
-
The titanium structure is designed to integrate seamlessly with soft tissue, minimising complications.

Frequently Asked Questions
-
Dental implants offer several benefits, including:
- Stability and Durability: They integrate with the bone in a natural way, providing a permanent solution.
- Improved Functionality: They allow chewing and speaking with the same efficiency as a natural tooth.
- Bone Preservation: They help prevent bone loss that usually occurs after tooth loss.
- Superior esthetics: They provide a natural appearance and improve the patient’s confidence and smile.
-
Yes, in many cases it is possible to place the implant on the same day of extraction, provided the bone and tissue conditions are adequate. This procedure is known as “immediate implant” and can reduce the total treatment time. However, the decision depends on a detailed clinical evaluation by the specialist.
-
The placement of a single dental implant is generally painless, as it is performed under local anaesthesia. During the procedure, the patient feels no pain, although it is possible to experience slight discomfort or swelling in the days following the procedure, similar to that experienced after a tooth extraction. This discomfort is usually mild and can be easily controlled with painkillers prescribed by the professional.
-
The duration of the entire treatment varies depending on the case. It can take between 3 and 6 months from the placement of the implant to the placement of the final crown. This time allows the implant to integrate properly with the bone (osseointegration). In some cases, if bone grafting or additional treatments are required, the process may take a little longer.
-
Although dental implants have a high success rate, some complications may arise, such as:
- Infection: May occur at the implant site if postoperative hygiene instructions are not properly followed.
- Implant Rejection: In rare cases, the body may reject the implant.
- Nerve Injury or Damage: May cause pain, numbness or tingling sensation in natural teeth, gums, lips or chin.
- Maxillary Sinus Problems: Especially in implants placed in the upper jaw, they can penetrate the maxillary sinuses.
Lack of Osseointegration: The implant does not integrate properly with the bone, which may require removal of the implant and placement of a new implant.
-
A single implant requires similar care to that of a natural tooth. It is essential to maintain good oral hygiene (brushing, flossing or using interdental brushes) and to attend regular check-ups with your dentist for implant maintenance. Remove your prosthesis on occasion.
It is also recommended to avoid harmful habits such as smoking or biting hard objects, as these can compromise the health of the implant.
